UnitedHealthcare may grant permission to create a link from an external
Web site to the unitedhealthcare.com home page. Links to pages within
the unitedhealthcare.com Web site are strongly discouraged due to
the evolving nature of the Web site.
OVERVIEW OF OUR POLICY
We encourage employers contracted with UnitedHealthcare to create
links to our member Web site, myuhc.com for their employee's online
self-service and wellness information needs. In addition, the Employer
eServices Web site should be used to handle administrative and other
supportive practices with current employers.
Upon receiving approval to link to unitedhealthcare.com, we will
send your business a unitedhealthcare.com Wordmark guide and corresponding
electronic file(s).
UnitedHealthcare is not responsible for your Web site, nor is it
promising any particular content or performance of its own site.
We may review your site from time to time, and we reserve the right
to withdraw our permission for this link at any time. Please remember
that this does not give you permission to use UnitedHealthcare's
copyrighted materials or trademarks that may appear on unitedhealthcare.com.
If you have any questions on what UnitedHealthcare materials you
may use, please contact us prior to beginning any unapproved use.
In addition, permission to link to unitedhealthcare.com does not
include a reciprocal link to your Web site. If you are interested

UnitedHealth Premium Disclaimer
The UnitedHealth PremiumSM designation program uses data from publicly
available data sets, claims and survey data from physicians to create
a multi-dimensional view of physician and facility performance.
Only individual physicians and facilities that meet UnitedHealth
Premium criteria, or physicians in designated specialties that are
part of medical groups that meet UnitedHealth Premium criteria for
group practices and who have sufficient claims data for analysis,
may be included. Group practice designations include evaluation
of administrative criteria and physicians are evaluated on an aggregate
basis. A group practice may qualify for designation even if an individual
physician within the group does not otherwise meet individual criteria.
Oncologists are not designated on a group practice basis. Physicians
in the network but not designated, have declined designation, did
not have adequate claims data or did not meet program criteria.
Oncologists without quality designations may have not applied for
designation or did not meet program criteria. Facilities listed
in the network but not designated, either have not applied for designation,
or did not meet program criteria.
This designation is intended as a resource for informational purposes.
UnitedHealthcare does not provide health care services nor practice
medicine. Physicians are solely responsible for medical judgments
and treatments. This designation does not guarantee the quality
of care being rendered. All physicians and facilities that contract
with UnitedHealthcare have met credentialing requirements. Regardless
of designation, plan enrollees have access to all physicians and
facilities in the UnitedHealthcare network.
UnitedHealth Wellness Portfolio Disclaimer
UnitedHealth Wellness is a collection of programs and services offered
to UnitedHealthcare enrollees to help them stay healthy. It is not
an insurance product but is offered to existing enrollees of certain
products underwritten or provided by UnitedHealthcare Insurance
Company or its affiliates to encourage their participation in wellness
programs. Health care professional availability for certain services
may be dependent on licensure, scope of practice restrictions or
other requirements in the state. Therefore, some services may not
be included in some programs due to state regulations. Some UnitedHealth
Wellness programs and services may not be included in all medical
plans for all customers and individuals.

Care24 Resources Disclaimer
Care24SM is a service mark of UnitedHealth Group, Inc., used by
permission. Due to the potential for a conflict of interest, legal

Definity Disclaimer
